What exactly is a superbug, and why do they scare doctors?

Doctors have medicines that can kill almost any harmful bacterium. But every so often a germ turns up that shrugs off medicine after medicine, and nothing seems to stop it. These are the ones that keep doctors awake at night. What makes an ordinary germ turn into a monster like this?
